
Intermediate Care and Reablement Project
Croydon place is looking at reviewing its intermediate care and reablement services to see how they can work better for patients. Currently patients who are discharged from hospital are assessed for either intermediate care services (provided by the NHS) or reablement services (provided by Croydon Council) which are assessed again after six weeks.
Intermediate care services tend to focus on helping patients by doing tasks for them and can take place at home by daily visits or in specific care facilities. Reablement services tend to focus on empowering the patient to become more independent through support and therapy.
Some services such as occupational therapy can be offered under both services. Croydon is reviewing these services and wants to understand patient need and expectations to support future commissioning to understand primary whether people would prefer to be in care facilities or to be supported at home. Afro-Caribbean experiences of Diabetes services survey
We are conducting a survey to learn more about the experiences of people from only African, Caribbean, and mixed heritage backgrounds, who have been diagnosed with type 2 diabetes. This includes when you were first diagnosed, how you access and use health services, any lifestyle changes you've made, and the outcomes you have experienced.
